Personality
Kaori is a free-spirited violinist whose playing style reflects her personality. She is independent, unpredictable and angry. It is also mentioned that she is sometimes passive aggressive. Kaori likes to depend on herself and only on herself, but she is very sociable and can be seen as a support to others.
Although considered a really sloppy violinist from a technical standpoint, such as not following the musical scores as they are written, Kaori is an incredible violinist. Noticed by one of the Towa Hall judges in the first competition of the series, her pure abilities far exceed the others in the competition. She exercises a free-spirited performance style that focuses on emotions and artistry rather than the pure mechanics of music, playing to specific tempos and dynamics originally intended by the composer. Her style of music is what reignites Kousei's passion for the piano and inspires her to redefine herself as a musician.
She loves candy and toys, so much so that when she was temporarily released from the hospital, her first order of business was to immediately drag Kousei with her to a store full of candy and toys.
Despite being very quick to anger, Kaori is a very kind and empathetic person, this can be seen throughout her interactions with Kousei as she often understood how he felt about his abusive mother and how he was afraid to play the piano, her words helped Kousei through those dark moments, her compassion helped him to break free from his depression. Her kindness is also shown whenever she is with children, as seen when she plays hopscotch with them and before that she begs Kousei to play the piano with other children to make them happy and finally when she was at the mall with Kousei, she saw a lost child and held the child's hand until his mother came to find her and was happy to see them reunited. Overall, despite her anger and somewhat abusive behavior towards Kousei.

Capacity
Like Kousei, she is an extremely talented musician. However, unlike most musicians in the series, her style is different. She frequently changed the rhythm, tempo and pitch of the song by styling freely. As a result, the song itself becomes a different melody. Her free style captivated the audience despite the disapproval of the judges. In her youth, she started playing piano inspired by Kousei's piano skills, but decided she wanted to play violin so that Kousei could accompany her. She still uses her piano skills by playing her melodica.

Biography
Context
When Kaori was a young girl, her class went to a music recital, where she saw Kousei Arima for the first time. As soon as he touched the keys and started playing, she was immediately inspired by his music.
Kaori was physically weak and underwent many surgeries. Despite this, her hospitalizations became more frequent and prolonged, often causing her to miss school and practice. One night, perhaps listening or just passing by, she saw her mother crying in the hospital lobby. She then discovered that she didn't have much time left to live, and soon after, she realized this herself through the recurring hospitalizations and developing weakness.
When she realized that she would be attending the same high school as Kousei, she wanted to approach her. Because Tsubaki Sawabe was a close friend of Kousei's, she found it hard to get his attention. So she developed courage, replaced her glasses with contact lenses (which she was too afraid to wear) and became herself, doing whatever she wanted. Knowing that Tsubaki was a close friend of Kousei, Kaori asked him to introduce her to Ryota Watari, another close friend of Kousei. This way, she could get closer to Kousei.
Shigatsu wa Kimi no Uso
Kaori is playing with three children, trying to summon pigeons by playing her melodica while standing atop a dome in the park. She catches a boy taking her picture as a gust of wind blows down her dress, and immediately hits him with her melodica and nails him with a recorder. Realizing that she had attacked the famous pianist Kousei Arima, Kaori apologizes for her violence and is introduced to Ryota Watari by Tsubaki Sawabe as she had requested. Kaori invites the three to watch her in the preliminary round of the violin competition. The three of them agree and leave for Towa Hall.
When it was Kaori's turn, she changed the setting ("Kreutzer" by Beethoven) and transformed it into her own original score. At the end of the performance, the audience applauded and cheered her tumultuously (much to the anger of the judge, who was not happy with what she played). After the end of the preliminary round, a girl gave her a bouquet of flowers. Kaori was elected "people's choice", taking her to the next round of the competition.
Kaori hid her illness from everyone. This was first shown when she collapsed after her first performance with Kousei. At that time, she claimed to be anemic. However, there were many suggestions for more serious health problems. She was shown medication during Kousei's competition and was unable to participate in her encore round with Kousei during her extended hospitalization. It was later revealed that she was unable to play with Kousei after falling and hitting her head at home. When Kousei heard about Kaori's hospitalization from Tsubaki, he rushed to visit her.
Later, it became clear that her health was deteriorating rapidly and that she could no longer hide her illness from her friends. Her illness began to cause episodes of paralysis in her legs. In addition, she suddenly fell to the floor while walking down the hospital hallway and was unable to get up. During one of her hospital visits, Kousei noticed that she had become paler and thinner.
As Kaori's surgery date approached, she wrote a farewell letter to Kousei. On the day of Kousei's piano competition, Kaori underwent surgery. But despite the doctor's efforts, she passed away on February 18 at the age of fourteen. At her funeral, her letter was given to Kousei by Yoshiyuki Miyazono and Ryouko Miyazono (Kaori's parents).
Relationships
Kousei Arima
Although Kousei had never met Kaori before, Kaori and her parents had always admired her since they first saw her play. Kaori began playing the violin in hopes of one day playing with Kousei. Throughout their time together, they had many instances in which Kousei slowly developed feelings for Kaori. With each encounter, Kousei realized the importance of himself as a musician, learned more about Kaori, and grew closer to her, which eventually turned into romantic feelings. However, Kousei refused to confess because he was misled into thinking that Kaori was in love with Watari.
When Kaori collapsed on the day of the gala due to the sudden paralysis of her legs and received a concussion, Kousei began to doubt himself, seeing his mother in Kaori, as the two had had a huge impact on Kousei's life, and the two were the reason why he played the piano. Kousei began to give up music again as he felt that music was the one taking away everything that was precious to him. However, after talking to Kaori about not giving up hope in the hospital, Kousei was touched by her words and decided to persevere once again in his final piano competition while Kaori was undergoing surgery. Unfortunately, Kaori could not make it through the surgery that led to her death, and she is likely dying on the operating table. Kousei received a letter from Kaori's parents written by Kaori before she underwent surgery. In the letter, Kaori expressed her love and gratitude to Kousei and thanked her for bringing joy and meaning to her life. At the end, Kaori wrote that she was in love with Kousei, not Watari.
Ryota Watari
Watari is Kaori's supposed love interest throughout the series. Both are very close friends and tend to be in love with each other. It is later revealed that she had a false interest in Watari just to get closer to Kousei. Watari immediately fell in love with Kaori's appearance when he first met her, but has always remained a ladies' man, chasing other girls at school. Nevertheless, he is shown to care for her and frequently visits her when she is in the hospital, bringing her gifts and making her laugh. In Kaori's letter to Kousei, she claims that Watari was an amazing friend, but he will probably forget all about her; however, it is visually suggested that he really loved her.
Tsubaki Sawabe
Tsubaki is a classmate of Kaori who asked her to introduce her to Watari. So, Tsubaki finally gave her the opportunity to meet Kousei. Kaori tried to meet Kousei in such a roundabout way because she knew that Tsubaki would not respond well if she was directly asked to be introduced; in her letter to Kousei, she mentioned that she knew that Tsubaki was in love with Kousei from the very beginning. Kaori and Tsubaki can be seen going home together throughout their time together, and they prove to be good friends. Tsubaki liked Kaori as a friend but realized her own feelings for Kousei after noticing that Kaori and Kousei's relationship was developing.
Anecdotes
In addition to being a violinist, Kaori has the ability to play the melodica and has some experience playing the piano.
Kaori had a rather sweet tooth because she liked sweet foods.
In "A single petal that drifted into my life" ( a quote from Kousei Arima), the petal is a metaphor for Kaori, who appeared out of nowhere and disappeared because of her fragility.
It is not known what disease Kaori suffered from, but fans of the series have speculated that she suffered from spinal cord failure, amyotrophic lateral sclerosis (ALS) or Friedreich's ataxia. Although Friedreich's ataxia is the most likely disease, because the symptoms correspond to the symptoms we see in the series.
